My first visit
We want every dental visit to be a great experience, starting with your first one. Here's everything you need before your first visit.
Please arrive 15 minutes prior to your scheduled appointment and plan to spend between 60–90 minutes in our office. You will receive a comprehensive oral examination and x-rays to help your dentist assess your oral health, looking for signs of tooth decay, bone loss, oral cancer, and Periodontal (Gum) Disease.
To save you time please bring the following things with you to your appointment: Dental Insurance Card (If applicable) and Photo ID. We will send you all forms electronically.
Financing & insurance
Speak with one of our friendly office members by call or text to help with any questions or concerns about your insurance plan, or the various payment options. We also offer a comprehensive membership plan (Click here for more information). We are happy to work with all patients to ensure that each patient gets the dental care needed to Smile With Pride™. Patient forms
We are so much more than your average dental office because we accept all gender identities, and everything can be filled out electronically. If you would rather fill out forms on a clipboard, you can! We will make getting started in our dental home easy.
